When requesting warranty service, please use the CS – Warranty Claim Form.
E-mail your completed Medical Product Warranty Claim Form to EMP at email@example.com or fax your completed claim form to EMP. Be sure to include photos and additional information as directed in the EMP Product Warranty Information Sheet.
Once a reported problem has been documented and investigated, a determination will be made regarding replacement or repair. Please allow two weeks for your warranty claim to be processed after submission.
Product warranty will be void if the product has been altered, used for purposes other than those stated, improperly maintained or subject to misuse, abuse or neglect.
